Nevada County is pleased to announce that we have started working to develop a 2024 Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP)!
A Community Health Assessment (CHA) will identify key health needs and issues in our community through comprehensive data collection and analysis. The Community Health Improvement Plan (CHIP) will build off that assessment by developing long-term goals, strategies, and activities to address public health concerns in Nevada County.
The CHA and CHIP will be used by the Nevada County Public Health Department and other governmental, education, and human service agencies, in collaboration with community partners, to set priorities and coordinate and target resources.
Through this Community Health Improvement (CHI) process, we will develop a 3 to 5-year plan that will serve as a road map for how we can increase healthy living and health equity throughout Nevada County!
Getting Involved
The CHI process is a collaborative process and each of us has a role in working to improve health and the quality of life in Nevada County. We want to develop a CHA and CHIP that include the voices of Nevada County’s diverse communities and that works to improve health for all our residents.
There will be many opportunities for you to be involved during this process, but right now we would like to get your feedback on the Nevada County Community Health Improvement Vision and Value Statements. To provide this valuable feedback, please complete this survey by July 21st:
