Community Health Improvement Project

Nevada County is pleased to announce that we have started working to develop a 2024 Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP)!

NEVADA COUNTY CHI (1)

A Community Health Assessment (CHA) will identify key health needs and issues through comprehensive data collection and analysis, while the Community Health Improvement Plan (CHIP) will build off that assessment by developing long-term goals, strategies, and activities to address public health concerns in Nevada County. The CHA and CHIP will be used by the Nevada County Public Health Department and other governmental, education, and human service agencies, in collaboration with community partners, to set priorities and coordinate and target resources.

Through the Community Health Improvement (CHI) process, we will develop a 3 to 5-year plan that will serve as a road map for how we can increase healthy living and health equity throughout Nevada County.

NCPH Vision for 2023 CHA/CHIP is An engaged diverse, connected, health and happy community.
