The Nevada County Senior Outreach Nurses provide health services to county residents 60 years and older. The nurses provide:
Secondly, they provide outreach such as community based blood pressure clinics, flu shots in conjunction with Public Health, and they participate in local health/wellness programs.
Senior Outreach Nurses receive referrals to see seniors from a variety of sources - self, family members, neighbors, physicians, and other service providers.
The program is designed to support seniors so they can live independently as long as possible, and avoid unwanted or premature placement in health care facilities. Nurses work with clients and other care providers to develop a plan of care. Follow-up visits are provided as needed to respond to changing needs of clients.
